All students must submit updated emergency forms each year. Select

Prior to the beginning of the school year, all students entering
kindergarten or grade 1 must have updated physicals and immunizations. Updated forms must be completed by your doctor
and returned to Saint Patrick School prior to the first day of school. The school health form may be downloaded by clicking
